Scheduling

Our typical office hours are Monday-Tuesday from 8:00AM to 4:00PM, and Wednesday-Thursday from 8:00AM to 12:00PM. COVID19 may alter our hours, and certain holidays or continuing education may cause temporary changes to this schedule. You can view our COVID policy and a list of closures. Please allow approximately ninety minutes for your initial visit, and be prepared to sign the COVID19 consent. For the latest office notifications, please click here.

Cancellation and Confirmation Policy

We understand that sometimes circumstances arise that may prevent patients from keeping appointments. However, we ask that you give a minimum of 48 hours business days notice if you need to make any type of appointment change for examination appointments, otherwise a $90/hr fee will be assessed. Business days do not include Friday through Sunday. The fee is $180/hr if you simply do not show up. For surgical appointments we require no less than 1 week notice, with a cancellation fee of $300/hr. This allows us the opportunity to give another patient valuable time with Dr. Nicoara. Confirmation is also required no less than 48hrs prior to your appointment. You will get a reminder on the day your appointment is made, and several days in advance to confirm.

Financial Policy

Fees for care are due at the time of treatment, and are your responsibility regardless of whether insurance pays for any of the charges. Your account becomes delinquent if not paid 30 days after billing, and at that time a finance charge of 1.5% of the unpaid balance may be charged every month for 6 months until the balance is paid in full, after which time you may be sent to collections. You can view or download the Privacy and Financial Agreement that you will sign at your initial visit.

We do not accept cash or checks.

Insurance

We are currently contracted with Delta Dental, MetLife and Recence insurance plans at this time. We are not contracted with any other dental or medical insurance providers. As a courtesy to patients with dental benefit plans, we will submit claims for care on your behalf. For any non-Delta plans, your insurance carrier will re-imburse you directly. We are a preferred provider with L&I.

We do not have post-treatment payment plans, but you can make pre-payments towards your care in advance. We are also participating with Care Credit, a medical credit card, which provides a 12 months no interest payment option through our office.

Online Forms and Bill Payment

Our practice is a state of the art facility using the latest digital technology for patient management and x-ray imaging. As a new patient, you will be required to complete a 3 page Patient Registration and Health and Dental History form on your initial visit. In order to save time at the office and to help us prepare for your visit, you may fill out and securely submit the 3 page form online by clicking the link below. Be sure to click the submit button when you reach the end. You will electronically sign the forms when you arrive at the office. You can click the Bill Payment link above to securely pay your bill online.
